When I only enable refresh, the width of the pager_left area seems to be calculated correctly.
I've also noticed that depending on how many options I add to the pager_left, the pager_right section gets pushed off to the right.
I've added the following and now I don't get overlap and the entire icon seems to be enable (tool tip pops up when hovering anywhere over the icon)
autowidth:true,
Still think there are gremlins in the width calculations but autowidth seems to make verything happy.
